Food Pantry Request Form

Please complete this form if you are a student requesting assistance from the 'Cougs Helping Cougs' food drive. Let us know what food items you are requesting and we will have a bag ready for you at either noon or 4 p.m. Requests will be handled in the order received and will be fulfilled based on stock on hand.

All food bags may be picked up at the Cougar Center front desk located on the upper floor of the Student Services building. Go to the front desk, tell the student employee your name and that you are here to 'pick up a food bag'. That is all that is needed for the pick up.

This program is dedicated to serving our students. We respect your situation, the confidentiality of it and will not ask for or disclose any personal information. We ask that students limit requests to one per week.

Please supply at least one method to contact you in case we have a reason to contact you, such as a delay in food or another time sensitive matter. We will not use this information for any other purpose.

3. Food Selection:

Check the items you would like to receive in your food bag. If you want just one of an item select it once; if you would like two (the max) please select both options for that item.

4. Pick-up Date & Time:

Our staff will work to put together the bags of food in a timely manner. Requests will be organized and food will be ready at noon and 4 p.m., Monday through Friday. We request at least a 2 hour turnaround to fulfill orders
