Auditory services absence notification

If you receive ASL interpreting services and are going to be absent from school, you must fill out this form in order to notify the WSU Vancouver Access Center and Clark College Sign Language Interpreter Coordinator.

This form must be submitted as soon as you are aware you will be missing class.

Access Equipment check out/drop off:
