I think I may have a disability

  1. Submit the Information Inquiry form.
  2. Once you have submitted the form, Access Center staff will contact you to schedule an informational meeting.

At the meeting, which will take about 30 minutes, the Access Center coordinator will discuss the steps you need to take to obtain documentation as well as your questions and concerns. Please print and fill out the Information Inquiry form before the informational meeting.

I have a documented disability

  1. Submit a Request for Services form and collect the appropriate documentation.
  2. Once you have submitted the form, Access Center staff will contact you to schedule an intake meeting.

Note: For Access Center to provide services, you must provide documentation of a qualified disability. Please review the Documentation Guidelines to determine what is required.

At this meeting, which will take about an hour, the Access Center coordinator will explain appropriate and reasonable accommodations. Please submit the Request for Services form beforehand and bring and your documentation to the intake meeting. The coordinator will discuss what services may be provided at WSU Vancouver and will review each accommodation that may be available to you, based on your qualified disability and documentation. Review the services offered. Once reasonable accommodations have been determined based on your documentation, the Access Center coordinator will provide a Letter of Accommodation (LOA) to be given to instructors.
